We provide our participants care and assistance with their day to day essentials such as personal hygiene, mobility, toilet and continence management.
Community Nursing Care
Our highly skilled nurses are available to provide their effective patient-centered and goal-oriented care to our participants in the community/home setting that is specifically modified for their needs.
Assist Travel / Transport
Our team assist participants to access their community by supporting them to develop their capacity to travel independently.
Group & Centre Based Activities / Community Participation
Activities and community participation are essential for the participants well-being, this enables them to develop a “safe space” and sense of acceptance within a community or group. Our team assist and supervise our participants to engage in a community, social, recreational, or economic activities.
Assist with Household Tasks
Participants may need assistance to maintain their living environment. Our team provide support to our participants in maintaining their living space by assisting them with household tasks and undertaking essential household chores that the participants’ is unable to undertake.
Daily Tasks / Shared Living Arrangement (SIL)
As a part of our holistic approach, our skilled support workers provide assistance and supervision with the tasks of everyday life of our participants. We offer aid and support to our participants and focuses on developing their skills to be able to live independently.
